What is a CPAP machine and how does it work?

CPAP stands for Continuous Positive Airway Pressure. A CPAP machine delivers a steady, continuous flow of pressurised air through a mask into the upper airway during sleep, acting as a pneumatic splint that holds the airway open and prevents the tissue collapse that causes apnea events. The therapeutic pressure — measured in centimetres of water (cmH₂O) — is prescribed by your sleep specialist based on your sleep study results, and typically falls between 4 and 20 cmH₂O.

What is the difference between CPAP, APAP, and BiPAP?

These three therapy modes represent different approaches to positive airway pressure delivery:

CPAP (Continuous Positive Airway Pressure) delivers a single, fixed pressure throughout the night. It is the most widely prescribed and extensively studied PAP therapy for obstructive sleep apnea.

APAP (Auto-titrating Positive Airway Pressure) — also called Auto CPAP — uses algorithms to continuously monitor airway resistance and automatically adjust the delivered pressure within a clinician-set range throughout the night. It reduces pressure when the airway is stable and increases it when obstruction is detected, resulting in greater comfort for many users compared to fixed-pressure CPAP.

BiPAP (Bilevel Positive Airway Pressure) delivers two distinct pressure levels — a higher pressure during inhalation (IPAP) and a lower pressure during exhalation (EPAP). This bilevel approach eases the work of breathing and is clinically indicated for patients with complex or severe OSA, central sleep apnea, COPD, neuromuscular conditions, or those who cannot tolerate the constant pressure of standard CPAP therapy.

Which device is right for me — CPAP, APAP, or BiPAP?

The correct therapy mode is determined by your sleep specialist based on your sleep study results, Apnea-Hypopnea Index (AHI), blood oxygen saturation, any comorbid respiratory conditions, and your individual response to therapy. What is the Apnea-Hypopnea Index (AHI)?

The AHI is the primary metric used to quantify sleep apnea severity. It represents the average number of apnea and hypopnea events per hour of sleep recorded during a sleep study:

  • Mild OSA: 5 to 14 events per hour
  • Moderate OSA: 15 to 29 events per hour
  • Severe OSA: 30 or more events per hour

Effective CPAP therapy aims to reduce the AHI to below 5 — ideally below 2 — which is within the normal range.

Do I need a prescription to buy a CPAP machine in the UAE?

Under current UAE regulations, a formal written prescription is not a legal requirement to purchase CPAP equipment. However, CPAP Medical Trading strongly recommends that all customers obtain a valid prescription from a licensed physician or sleep specialist before purchasing any PAP therapy device. CPAP and BiPAP devices are prescription medical devices in clinical practice — using them at incorrect pressure settings without medical guidance can be ineffective at best and potentially harmful at worst.


Choosing the Right CPAP Mask

What types of CPAP masks are available?

CPAP masks are available in three main configurations:

Full face masks cover both the nose and mouth. They are recommended for mouth breathers, patients with nasal congestion, those requiring higher therapy pressures, and patients prescribed BiPAP therapy. They offer the most stable seal and are the most commonly prescribed mask for BiPAP users.

Nasal masks cover only the nose and are the most widely prescribed mask style overall. They suit active sleepers, patients who move during the night, and those who breathe comfortably through the nose throughout sleep.

Nasal pillow masks sit at the entrance to the nostrils rather than covering the nose. They are the most minimal and lightweight option — preferred by patients who experience claustrophobia with larger masks, those who wear glasses, and side or stomach sleepers.

Why does my mask leak air, and how do I fix it?

Mask leak is one of the most common issues in CPAP therapy and is almost always solvable. The most frequent causes are a worn or degraded cushion that has lost its seal, a mask that is the wrong size or style for your facial anatomy, and headgear that has stretched over time and no longer holds the mask in proper position. Check that your cushion is due for replacement (every 2 to 4 weeks for most cushion types), ensure the headgear is adjusted to a snug but not overtight fit, and consider a mask fitting review with our team if the issue persists.


CPAP Humidifiers & Water

Do I need a CPAP humidifier in the UAE?

Yes — humidification is particularly important for CPAP users in the UAE. Air-conditioned environments, which are almost universal in UAE homes and hotels, produce very low ambient humidity. Combined with the region's naturally arid desert climate, this makes airway dryness, nasal congestion, and throat irritation during CPAP therapy more likely than in more temperate climates. Most modern CPAP and BiPAP devices include integrated heated humidifiers, which are the most effective solution. We strongly recommend using the humidifier function.

What water should I use in my CPAP humidifier?

Always use distilled water — and only distilled water — in your CPAP humidifier chamber. UAE tap water and bottled mineral water both contain dissolved minerals that accumulate in the chamber and heating element, causing scale build-up that reduces humidifier performance, shortens the life of the water chamber, and may harbour bacteria. Cleaning & Replacing CPAP Supplies

How often should I clean my CPAP equipment?

Daily: Wipe the mask cushion with a damp cloth or CPAP mask wipe to remove skin oils. Empty remaining water from the humidifier chamber, rinse with clean water, and allow to air dry completely.

Weekly: Disassemble the mask fully and wash all components (except headgear) in warm water with a small amount of mild, non-antibacterial soap. Rinse thoroughly. Wash the tubing by running warm soapy water through it, then rinse and hang to dry. Clean the humidifier chamber in the same way.

Never use bleach, alcohol, antibacterial soaps, washing machines, tumble dryers, or dishwashers on any CPAP component. These degrade silicone materials and leave residues that can irritate the skin and airways.

How often should I replace CPAP supplies?

Worn cushions are the most common cause of mask leak, which directly compromises the therapeutic pressure reaching your airway. Do not delay cushion replacement when you notice air escaping around the seal.

Can I use an ozone or UV CPAP sanitiser?

Ozone-based CPAP sanitisers use activated oxygen to kill bacteria and eliminate odours, and UV devices use ultraviolet light for similar purposes. Both can be useful as a supplementary hygiene measure. However, neither replaces regular manual cleaning with warm water and mild soap, as they do not remove the physical residue of skin oils and mineral deposits that accumulate on mask surfaces and tubing over time. Check your device and mask manufacturer's documentation before using an ozone cleaner, as some manufacturers' warranty terms exclude damage caused by these devices.


Travelling with CPAP

Can I take my CPAP machine on a flight from the UAE?

Yes. CPAP and BiPAP devices are classified as medically necessary equipment by the International Air Transport Association (IATA) and are permitted as additional carry-on items by most major airlines, including Emirates, Etihad, flydubai, and Air Arabia — beyond standard cabin baggage allowances. We recommend carrying a copy of your prescription or a physician's letter confirming medical necessity, as some carriers require advance notification for medical devices.

Do I need distilled water for my humidifier when travelling within the UAE?

Yes. UAE tap water contains dissolved minerals that are not suitable for CPAP humidifier chambers, even if it is safe to drink. When travelling between emirates or staying in hotels across the UAE, always use distilled water in your humidifier. Starting CPAP Therapy

Is it normal to struggle with CPAP therapy at first?

The first 90 days are the most critical period for establishing therapy adherence. How long do I need to use my CPAP machine each night?

CPAP therapy is most effective when used for the full duration of sleep every night, including naps. Clinical guidelines define minimum adequate adherence as four or more hours of use per night on at least 70% of nights, though the optimal cardiovascular and neurocognitive benefits of therapy require consistent full-night use.

Your device records compliance data — including nightly hours of use, residual AHI, leak rate, and delivered pressure — which your physician can review at follow-up appointments to assess therapy effectiveness and make any necessary adjustments.

What should I do if my CPAP therapy does not seem to be working?

If you continue to experience daytime sleepiness or unrefreshing sleep after several weeks of consistent use, do not assume the therapy is ineffective before speaking to your physician. Common causes of apparent therapy failure include: an incorrect or outdated pressure setting, significant mask leak reducing delivered pressure, mouth breathing through a nasal mask, or untreated co-existing conditions such as insomnia or restless leg syndrome. Results are typically generated within 24 to 48 hours of the test night.

Should I see a doctor about my home sleep test results?

Yes — we strongly recommend it. While the WatchPat One generates a detailed sleep study report, the results are raw clinical data that require interpretation by a qualified physician or sleep specialist before any treatment decisions are made. A doctor will review your Apnea-Hypopnea Index (AHI), oxygen saturation data, and overall sleep profile to confirm a diagnosis, rule out other conditions, and determine the most appropriate therapy — including the correct device type and pressure settings for your specific case.

CPAP Medical Trading does not interpret sleep study results or prescribe therapy. We can assist you in selecting equipment based on a prescription your physician provides following their clinical review of your results, but the medical interpretation step must always come first.

Is a home sleep test as accurate as a laboratory sleep study?

Home sleep tests such as the WatchPat One are validated for diagnosing moderate to severe obstructive sleep apnea in adults without significant comorbid conditions, and are widely used across the UAE as a first-line screening tool for suspected OSA.

Full laboratory polysomnography (PSG) remains the clinical gold standard and is required when OSA is suspected alongside other sleep disorders — such as periodic limb movement disorder, narcolepsy, or parasomnias — or when a home test result is inconclusive.
